It has been some time ago, but it occurred if you took the quest to help trist without having made Byron Pikit angry with you or made him send you to Lenny. Then instead of going directly to Lenny you go talk to to Pikit and convince him to send you to see Lenny i.e. he thinks you are on his side. Then you talk to Lenny he (if you tell him you are on Trist's side) will send you to the warehouse to fetch the evidence he did not destroy, but when you arrive at the warehouse you will not get the conversation option, which will make the vault give you the evidence.
